Prepare For Your Visit
In preparation for your visit to Midwest Ear, Nose & Throat Specialists, please make a note of the following:
- Bring all insurance information: we will need to make a copy of your insurance card(s).
- Co-payments are due at the time of your visit: We accept cash, checks, MasterCard, Visa and Discover.
- Bring all recent lab work and x-ray films: Failure to do so may delay your appointment.
- Obtain a referral from your primary care physician: If your insurance plan requires a referral to see a specialist, please contact your primary care physician to verify that a referral has been issued to us for your visit. If a referral is needed and not received, it may cause a delay and possible cancellation of your appointment.
- Please review the HIPAA Privacy Notice and No-Show Policy.
Completion of Paperwork/Forms:
At the time of your visit, you will be asked to complete our registration form. For your convenience, you may complete this form prior to your visit and bring it with you. All new patients are required to fill out the registration form as well as patients who have not been seen in the past year. Medication and Allergy sheets should be updated every visit. You still may need to fill out additional forms or provide further information at the time of your exam.
YOUR INSURANCE CARD AND PHOTO ID WILL BE REQUIRED AT EVERY VISIT FOR A NEW OR ESTABLISHED PATIENT.
